Story related achievements in Gears of War 4 are glitched for some profiles if you have played some parts on the PC.

New accounts don't seem to have that problem. If you have an older account it might be a problem to unlock one of the following achievements:

• Welcome Home
• And You Said We Couldn’t Fly
• As If Snatchers Weren’t Bad Enough
• Betty’s My Kind of Girl
• Broadcast This!
• Looks Like an Elevator to Hell
• One Hell of an Exhibit
• Raiding is what Outsiders Do
• Still Not Ironic
• Warriors, Kings….Wizards.

Even if you play the game on the XBOX ONE after that you won't be able to unlock them. It doesn't matter if you play solo or coop. It won't unlock.

I created a new account and they unlocked without any problems. Resetting and reinstalling didn't work. Even if you overwrite your cloud save with a blank profile these achievements won't pop.

After the announcement of Gears 5 The Coalition they didn't want to hear about problems in Gears of War 4 anymore. They said they might come back later. They announced one more update in January.

So I would like to get their attention to get a fix for this problem at least in January too. It might be the final update.
